    Here is the Structure of Your ICD-9 Codes Code Book!

    1.         Make sure you choose the most appropriate diagnosis code based on the
    medical documentation
    2.         Code on its highest specificity!
    3.         Secondary Diagnosis Code must also be coded on the HIGHEST SPECIFICITY!
    4.         Make sure you have read the ICD-9-CM Official Guidelines for Coding and
    Reporting Effective October 01, 2011 (this is the most current guidelines as of
    today as I am posting the page on February 07, 2013). There are no changes in 2012.

    The Volume 1 on the ICD-9-CM Coding Book pertains to the NUMERIC listing of the
    diseases, disorders, symptoms, by body system, anatomical site and location,
    underlying conditions, causes of injury, other reason of encounters and also personal
    and family history.

    After consulting with Volume 2 (Alphabetic Index) of your coding book, you will proceed
    to Volume 1 to verify your your code (s) and get more additional information based on
    the determined reasn for the encounter.
